Pulling the record is the easy part; reading it usefully is harder. Look at the deficiency narratives, not just the count of citations -- a missing signature and an injured resident can appear formatted identically in the same report. Watch for the same finding repeating across consecutive surveys, which suggests a pattern rather than a one-time lapse. And remember the ratings gap: a facility can have glowing online reviews and a troubling inspection history at the same time, because reviews measure customer service and the state survey measures regulatory compliance -- two different things entirely.
